The Monroe County Purple Tube Project is to provide First Responders and Emergency Personnel with important information about the medical and behavioral needs of individuals living with Alzheimer’s disease and dementia.

There are 2 ways that you can register a loved one for the Monroe County Purple Tube Project.  You can fill out the digital registration form below or download the form and fill it out and return it to the Dementia Care Specialist.
